Getting it:
Go to the sewers under St. Delyn in Vivec and look for a door with a guy in front. Kill him and open the door. There are a few guys inside, be sure to take care of them and then click on Sheogorath's Statue.
Sheogorath tells you that he needs you to go north to an island west of Dagon Fel. On the island is an Argonian named Big Head in his shack. He needs you to kill a giant bull netch there with the Fork of Horipulation, which you can find in the shack. Then come back to him and get the Spear of Bitter Mercy.
